Sense key Description
0B/4E/00 The LUN has encountered an "Incorrect Initiator Connection." Examples include
receiving two CDBs for the same I_T_L_Q nexus, or the mixing of tagged and
untagged CDBs for a given I_T_L_Q nexus (with no outstanding Contingent
Allegiance). Per the SCSI-2 specification, the SP will abort all outstanding CDBs
for the I_T_L nexus.
