6. On the Sample Resources window, make sure that the
Print Form Name check box in the Document Options
section is deselected.
7. Make any further desired changes to the other fields
on this window. For example, you can print multiple
copies by increasing the number in the Quantity box.
8. Click OK to print the test form.
