13. Return of Equipment

All requests for repairs and replacement parts should be directed to the Customer Service
Department at QSonica following the outlined procedure below.
1. Go to www.sonicator.com and click on the Contact Us section. Follow the troubleshooting
procedures. If troubleshooting does not fix your problem you must fill out a Repair Form.
This form helps us diagnose the equipment and speeds up the repair process.
2. After submitting a Repair Form online we will contact you with a RMA (Return Material
Authorization).
3. No returns are accepted without an RMA number.
4. When sending in equipment, always include the generator, converter, cables and horns.
5. Equipment should be returned in the original packaging.
Packaging
If you don't have the original packaging, it can be purchased by ordering the following:
Foam: # U2001A4000, U2002A4000 (both are needed)
Box: # U2000A4000, U2000A0CL4 (both are needed)
The total cost for the boxes and foam is $50.00 including shipping to any US location.
QSonica is not responsible for shipping damage on equipment returned to our facility for
repair. It is highly recommended that you purchase approved packaging if you do not have
your original box. If you choose not to use original packaging, you must pack each item
separately with substantial amounts of foam and packing materials to prevent damage.
Damage that occurs from shipping is not covered by the unit's warranty.

AGENT THUS CERTIFIES THAT ANY AND ALL MATERIALS SO RETURNED ARE, OR
HAVE BEEN RENDERED, FREE OF ANY HAZARDOUS OR NOXIOUS AND/OR
INFECTIOUS MATTER OR RADIOACTIVE CONTAMINATION AND ARE SAFE FOR
HANDLING UNDER NORMAL REPAIR SHOP CONDITIONS. DO NOT RETURN ANY
MATERIAL, FOR WHICH SUCH CERTIFICATION CANNOT BE MADE WITHOUT PRIOR
APPROVAL FROM QSONICA
