No idea what this page is asking for, but it is likely to be preferences/gender etc. Fill each box with a
random word (I did them all as "Japan") and select either radio button at the bottom.
This is your address. The first box contains 3 numbers (123), the second is a drop down box of
something or other. Choose the first three character option in the list. The last four text boxes are your
address, so fill them with random words again (Once again, I just repeated the word Japan).
