fill out the card and mail it at your earliest con-
venience. This will ensure that Zenith can locate
you promptly if we discover a safety problem
that would affect you.
If the original card has been lost or misplaced,
use the replacement card below. Either card will
qualify you for the free drawing, but you are lim-
ited to only one entry in the drawing. Complete
