State, area, organization, or group the asset belongs to (ex: North Region, MN tankers, etc.)

Gateway ID of each device to be installed (Gateway ID is found on the front of each unit with a
barcode)
•
ATG model
•
Number of Tanks
Required Equipment
ATG LMU Gateway
Serial Adapter
Required Tools
Drill and assorted drill bits (to secure device)
SMALL slotted screwdriver (for attaching serial cable)
