name, complete address, model and serial number along with your check or money order for
$25.00 payable to: Victor Technology, Attn: Extended Warranty Dept., 780 West Belden,

VICTOR will acknowledge receipt, send you an authorized coverage agreement, and provide the address of
the authorized VICTOR Regional Service Center nearest you.
Repair necessitated by accident or abuse is excluded.
If repairs are needed during the coverage period, ship your VICTOR calculator to the nearest authorized VICTOR Regional Service
Center, freight PREPAID. It will be promptly repaired and returned to you freight prepaid.
