Limited Warranty; Declaration Of Conformity - Lanaform TS1 Manual

LIMITED WARRANTY

LANAFORM® guarantees that this product is free from defects in material and
workmanship from the date of purchase and for a period of two years, with the
exception of the cases listed below.
The LANAFORM® warranty does not cover damage resulting from normal wear
of the product. In addition, the warranty over this LANAFORM® product does
not cover damage caused by abusive or inappropriate use or any form of misuse,
accident, fixing of any unauthorized accessory, alteration to the product or any
other condition , of any nature whatsoever beyond the control of LANAFORM®.
LANAFORM® shall not be held liable for any type of incidental, consequential or
special damage.
All implied warranties of fitness of the product are limited to a period of two
years from the date of original purchase, provided a copy of proof of purchase
can be given.
Upon receipt, LANAFORM® will repair or replace, as appropriate, your device
and return it to you. The warranty is only valid through the LANAFORM® Service
Center. Any maintenance activity of this product entrusted to any person other
than the LANAFORM® Service Center voids this guarantee.

DECLARATION OF CONFORMITY

We, LANAFORM,
Hereby declare that the medical device "Blood Pressure Monitor TS1"
complies with European Directive 93/42/EC on medical devices and the last
amendment 2007/47/EC.
APRAGAZ 156 chaussée de Vilvoorde 1120 Bruxelles (Belgique), certified
body N°0029, has made the product verification (according to annex IV)
