SYSTEM COMPONENTS (Save for packaging and return mailing.)

• Prepaid return shipping box with
pre-attached USPS mailing label
(return postage is prepaid)
• Inner box and 4 mil plastic bag liner
• Red sharps collection containers with
absorbent pad inside

INSTRUCTIONS (Read instructions including all warnings completely before using this system.)

Fig. 1: Remove container from
packaging and place at point
of use. DO NOTREMOVE
ABSORBENTPAD.
Fig. 5: When ready to mail,
place container into plastic
bag and original shipping
box. ONLY MAIL CONTAINER
IN ITS ORIGINAL BOX.

from plastic pouch on shipping
box. Complete top section.
YOU MUST SIGN THE FORM.
Remove bottom copy and
keep. Place remaining copies
back in pouch with barcode
facing out. Box will not be
accepted without tracking form.

ADDITIONAL WARNINGS:

• Maximum return weight including packaging listed on the shipping box
• KEEP AWAY from unauthorized persons
• Do not ship containers in any other box than the one provided
• NO mercury or lead-containing devices or batteries in this container
• NO more than 50ml (1.66 ounces) of liquids in any sharps collection container
• NO drugs or trace chemo in this container unless program previously established. Call Sharps at 800-772-5657 for proper
labeling for these materials.
• Comply with all laws and regulations. New Jersey regulated generators attach NJ Tracking Form to Sharps Tracking Form

necessary. Total package may
not weigh more than the weight
listed on the shipping box.
