product is pre-printed with its Model and Serial numbers.
Please fill out the card and mail it at your earliest convenience.
It is imperative that Zenith know how to reach you promptly if
we discover a safety problem that would affect y ou. If the
original card has been lost or misplaced, use the replacement
card below. Either card.will qualify you for the free drawing,
but you are limited to only one entry in the drawing. Complete
