Biopsy examination

5.1

Biopsy - general

The biopsy device allows for flexible applications. To localize lesions,
it can be used with the post/pillar as well as the grid method. In
addition, different accesses can be selected (medial/lateral or cranio-
caudal).
The following biopsy methods are used frequently:
Wire localization (open biopsy)
Vacuum biopsy (fully MR guided)
Setting of clips

Procedure

5.1.1
Access to the lesion to be biopsied is based on diagnostic pre- and
post-contrast images of the immobilized breast. Based on the images,
the coordinates of the lesions and/or their position in the biopsy
device (grid or post/pillar) are determined, and access to the lesions is
planned.
For wire localization a thin, core needle device (18G/20G) with
inserted marking wire is introduced into the breast. After the control
scan, the needle device is removed while the marking wire remains
until an open biopsy is performed on the lesion. Correct positioning
of the hook ensures free patient movement, eliminating the need for
positioning shortly before the actual operation.
When performing a vacuum biopsy, a combination of introducer
sheath and stylet is inserted into the breast. When the calculated
lesion position has been reached, the stylet is replaced by a synthetic
needle (obturator) followed by a control scan. Subsequently, the
biopsy is performed through the core needle device.
Clips can be deployed through the introducer sheath (depending on
the biopsy device marker) after the biopsy or similar to wire
placements.
