Bad contact to the

Initial battery voltage is
too low.
The positive (+) and
negative (-) polarity
terminals of the battery
are oxidized or dirty.
The battery has
been damaged.
The battery is too old.
The battery is
fully charged.
Improper placement.
Damaged charger.
