Important Safety Information: Retrofit of new bolts required to all Action 2000 wheelchairs
prior to manufacture 1 September 2008.
In the interests of the safety of our customers, we would like to bring your attention to possible

loosening of the castor bolts which are part of the front castor assembly on Action 2000. There
are two fixing bolts on each castor housing (see diagram on page 2).
In August 2007 the method of mounting the castor assembly to the sideframe was changed. The
thread length on the bolts then used to mount the castors has been found to be insufficient to
guarantee that the bolts can be correctly tightened.
Loose bolts (Slackened / Unwound during use, which has occurred over time) could potentially
cause the sudden failure of one or both of the castor housing bolts, and the possibility of a fall from
the wheelchair which could result in injury to the user.
The number of incidents remains low but we intend to strengthen the fixation of the castor housing
with a new bolt with loctite patch so that end users' safety is assured.
A modification has been implemented starting from the 1st September 2008, from serial number

7008568014 for brand new products. Therefore wheelchairs manufactured after 1 September
2008 are not affected by this Field Safety Notice
We recommend the castor mounting bolts on all wheelchairs within the affected range are changed
and we advise you to contact Invacare to obtain a free of charge replacement kit (Reference
1528509 – 4 * M6 Bolts with Fixing Instructions). Contact details are given below.
