It is hereby certified that the pulley block covered by this test certificate has been tested with the stated
proof load and subsequently examined by a competent person. The rated capacity is the maximum
permissible working load which should not be exceeded. Instructions with regard to the use and
maintenance of this equipment can be found in the relevant operating and maintenance manual,
which is supplied separately. Product liability insurance is in effect within the U.K. through Axa
Insurance Company policy number NHCOM 1193182 effected by Jack Sealey Limited.
