Health risk due to contaminated ultrasound bath
– Decontaminate the ultrasound bath before shipping if it has
come into contact with hazardous substances.
If the ultrasound bath needs to be repaired, send it to the manufac-
turer.
Clean the ultrasound bath before shipping it.
Decontaminate the ultrasound bath if it has come into contact with
toxic, corrosive, radioactive or biologically hazardous substances.
Also clean and decontaminate any accessories that you send in.

Fill out the form and attach it to the outside of the
packaging so that it is clearly visible. Acceptance
will be refused without a completed form.
