- Page 3 First Name, Last Name, Birth Date, Driver’s License Number, and Zip Code from the scanned ID. The First Name field must be selected in order to use the 2D scanner on this window. - Page 4 First Middle Name, Last Name, Gender, and Birth Date from the scanned ID. The First & Middle Name field must be selected in order to use the 2D scanner in this window.
