Mix75/25™ KwikPen
75% insulin lispro protamine suspension and
25% insulin lispro injection (rDNA origin)
Read the Instructions for Use before you start taking HUMALOG Mix75/25 and each
time you get a refill. There may be new information. This information does not take
the place of talking to your healthcare provider about your medical condition or your
treatment.
Do not share your HUMALOG Mix75/25 KwikPen with other people, even if
the needle has been changed. You may give other people a serious infection
or get a serious infection from them.

HUMALOG
Mix75/25™ KwikPen
("Pen") is a disposable pen containing 3 mL
®
(300 units) of U-100 HUMALOG
Mix75/25™ [75% insulin lispro protamine
suspension and 25% insulin lispro injection (rDNA origin)] insulin. You can inject
from 1 to 60 units in a single injection.
This Pen is not recommended for use by the blind or visually impaired
without the assistance of a person trained in the proper use of the product.

Attach Needle
Pull the pen cap straight off and wipe the rubber seal with an alcohol swab.
Mix Insulin
Gently roll the pen ten times and invert it ten times to mix the insulin.
Priming your HUMALOG Mix75/25 KwikPen
Prepare Needle
Pull the paper tab from the outer needle shield and remove the needle.
Secure Needle
Push the capped needle straight onto the pen and turn until tight.
Remove Needle Shields
Pull off the outer needle shield (keep it) and the inner needle shield (throw it away).
Set Dose for Priming
Turn the dose knob to select 2 units for priming.
Collect Air Bubbles
Hold the pen with the needle up and tap the cartridge holder to collect air bubbles.
Selecting your dose
Prime Pen for Injection
Push the dose knob in until it stops, see '0' in the window, and count to 5.
Select Your Dose
Turn the dose knob to select the number of units, aligning the Dose Indicator.
Giving your HUMALOG Mix75/25 injection
Choose Injection Site
Choose an injection site (stomach, buttocks, legs, arms) and wipe with an alcohol swab.
Insert Needle
Insert the needle into your skin.
Inject Dose
Push the dose knob in until it stops and count slowly to 5.
After your injection
Remove Needle
Pull the needle out of your skin. Check the dose window shows '0'.
Replace Outer Shield
Carefully replace the outer needle shield.
Dispose of Needle
Unscrew the capped needle and throw it away safely.
Replace Pen Cap
Replace the pen cap by lining up the cap clip with the dose indicator.
Disposal and Storage
Sharps Disposal
Dispose of used needles in an FDA-cleared sharps container or a safe household alternative.
Store HUMALOG Mix75/25 KwikPen
Store unused pens refrigerated and current pen below 86°F. Do not freeze.
General Safety Information
Keep pens and needles out of reach of children and always use a new needle for each injection.
