INSTRUCTION MANUAL INTRODUCTION Thank you for purchasing the StimChoice 4000 for your pain relief and muscle training solution. In order to use the device safely, read the complete manual carefully before using for the first time. Keep this instruction manual in a convenient place, or store with the device for future reference. - Page 5 INSTRUCTION MANUAL HOW TENS WORKS There is nothing “magic” about Transcutaneous Electrical Nerve Stimulation (TENS). TENS is intended to help relieve pain. The TENS unit sends comfortable impulses through the skin to stimulate the nerve (or nerves) in the treatment area. In many cases, this stimulation will greatly reduce or eliminate the pain sensation the patient feels.

INSTRUCTION MANUAL HOW TO CONTROL AND REDUCE YOUR PAIN WHEN SHOULD THE DEVICE BE USED? Use as soon as your pain begins. Start with one session. If you address your pain early, it may prevent the pain from becoming worse, or even chronic. It is better to get pain under control before it increases and may limit your daily activities. -

INSTRUCTION MANUAL INSTRUCTIONS FOR USE BATTERY 1. Remove the battery cover on the back of the device as shown as shown in Figure 1. 2. Insert (3) AAA batteries. Make sure the positive + and negative – signs correspond with the markings in the device when inserting batteries. -

INSTRUCTION MANUAL START THERAPY NOTE: Before proceeding to this step, be sure the device is completely turned OFF. STEP 1 Attach Electrodes to the Lead Wire Each wire has two leads with pin-inserted connectors. Insert the lead wire connector into electrodes connector. Make sure no bare metal of the pins are exposed. - Page 22 INSTRUCTION MANUAL STEP 4 Turn on the Device Press to turn the device on. The device will start showing all icons for 1 second to ensure that all options are working properly. STEP 5 Selecting the Treatment Mode Category To choose a treatment mode, press one of the below buttons on the device.

INSTRUCTION MANUAL ELECTRODE PLACEMENT GUIDE Please note that the electrode placement guide are only SUGGESTED electrode placements to help guide you in achieving successful therapy. The main points to remember when placing your electrodes are: • Read all cautions and warnings regarding electrode placement in the front of your StimChoice 4000 user manual. - Page 25 INSTRUCTION MANUAL WRIST / HAND Attach one pair of electrodes on the hand where you feel pain. ELBOW Attach one pair of electrodes on either side of the joint with the pain. CAUTION: DO NOT place electrodes on the bony premises and ensure electrodes are not lifting in any area.

INSTRUCTION MANUAL CLEANING AND STORAGE KEEPING ELECTRODES CLEAN Ensure the device is completely turned off when removing the electrodes from the treatment area. Disconnect the electrodes from each lead wire and place on protective liner supplied with the electrodes. If electrodes are difficult to attach to the skin or the protective liner, they may still be have more uses left prior to replacing. - Page 32 INSTRUCTION MANUAL STORING DEVICE, ELECTRODES AND LEAD WIRES After electrodes have been removed from treatment site and disconnected from lead wires, place electrodes on the plastic liner and store in a resealable package. Wrap lead wires and keep in the storage bag. If device is not being used for long periods of time, remove batteries from device before storing.
