Defective (leaking) fuel injector
Low charge air pressure
Excessive exhaust back pressure
Plugged intake port(s)

Air in fuel system
Faulty fuel injector
Leaking cylinder head gasket
Damaged intake or exhaust valves
Incorrect injection pressure
