User Name — Enter your FaxPress login name.
Password — Enter your FaxPress login password.
Cover Page Name — Enter a unique name for the cover page, not the same
name as an existing master cover page.

When you click on OK, you log into the specified FaxPress server with the specified
name and password; the cover page converts to fax-ready format and appears in
the appropriate folder.
